Job description:Nurses are responsible for providing education, training, case management and support for the Home end stage renal disease (ESRD) patients. The home nurse trains patients to assume primary responsibility for their own health maintenance, orient patients to the home program stage renal disease (ESRD) patients , and train the patient in home end stage renal disease (ESRD) patients techniques. Additional responsibilities developing and maintaining nursing care plans in collaboration with nephrologists, the Clinical Nurse Manager and/or other Home team members, by performing and documenting ongoing patient assessments, providing home training and therapy in center, and making periodic visits to the home to monitor and provide oversight of patient’s adaptation to home treatment. Job requirements:
- Current RN license in the state of employment
- End stage renal disease (ESRD) patients experience
- Current CPR certification
- At least 1 year of hemodialysis nursing experience
- Training/teaching experience/background encouraged— preferably in a home therapy environment including experience in Peritoneal Dialysis, Home Hemodialysis, Short Daily HD, and/or Self-Care HD
- Case management experience preferred
